{"id":2331,"date":"2022-04-28T09:50:30","date_gmt":"2022-04-28T08:50:30","guid":{"rendered":"https:\/\/www.djoglobal.eu\/blog_eu\/?p=2331"},"modified":"2022-04-28T09:50:31","modified_gmt":"2022-04-28T08:50:31","slug":"how-bracing-can-help-manage-ankle-sprains-and-instability","status":"publish","type":"post","link":"https:\/\/enovis-medtech.eu\/blog_eu\/?p=2331","title":{"rendered":"Managing ankle sprains and chronic ankle instability with bracing"},"content":{"rendered":"\n<p>An ankle sprain is one of the most common injuries experienced in physically active populations<sup>1<\/sup> and one of the most frequent sport-related injuries,<sup>2<\/sup> with indoor\/court sports having the highest incidence.<sup>3<\/sup><\/p>\n\n\n\n<p>Such injuries can result in the ankle joint becoming abnormally loose, which can then lead to chronic ankle instability (CAI). The pain, stiffness, and loss of confidence associated with CAI can be a major hindrance to physical activity, and the condition may require surgery to remedy. <\/p>\n\n\n\n<p>However, functional ankle braces offer patients a conservative way to manage both acute and chronic ankle injuries. This article examines the evidence for these products.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What is an ankle sprain?<\/strong><\/h2>\n\n\n\n<p>Lateral ankle sprains can occur due to forceful ankle plantarflexion and inversion,<sup>4<\/sup> when the foot suddenly rolls inward, resulting in damage to the ligaments of the ankle. In addition to pain, the patient may experience swelling, bruising, and additional discomfort when attempting to put weight on the foot.<\/p>\n\n\n\n<p><strong>There are three categories of sprain, increasing in seriousness:<\/strong><\/p>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<ul class=\"wp-block-list\"><li><strong>Grade I (Mild):<\/strong>&nbsp; Features slight stretching and microscopic tearing of the ligament fibers, resulting in mild tenderness and swelling around the ankle<\/li><li><strong>Grade II (Moderate):<\/strong> Partial tearing of the ligament occurs, resulting in moderate tenderness and swelling around the ankle. When the ankle moves in certain ways, there is an abnormal looseness of the ankle joint<\/li><li><strong>Grade III (Severe):<\/strong> The ligament is completely torn, resulting in significant tenderness and swelling around the ankle. When the ankle moves in certain ways, substantial instability occurs<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What is chronic ankle instability?<\/strong><\/h2>\n\n\n\n<p>40% of those who experience an ankle sprain go on to develop <strong>chronic ankle instability (CAI)<\/strong> and complain of chronic symptoms.<sup>5,6<\/sup> This is a condition characterized by the feeling, or the actual occurrence, of the ankle giving way.<sup>7<\/sup><\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<figure class=\"wp-block-image size-large is-resized\"><a href=\"https:\/\/www.djoglobal.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram.png\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.djoglobal.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram-1024x858.png\" alt=\"Grades of ankle sprain\" class=\"wp-image-2347\" width=\"512\" height=\"429\" srcset=\"https:\/\/enovis-medtech.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram-1024x858.png 1024w, https:\/\/enovis-medtech.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram-300x251.png 300w, https:\/\/enovis-medtech.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram-768x643.png 768w, https:\/\/enovis-medtech.eu\/blog_eu\/wp-content\/uploads\/2022\/04\/Ankle-sprains-diagram.png 1310w\" sizes=\"auto, (max-width: 512px) 100vw, 512px\" \/><\/a><\/figure>\n<\/div>\n<\/div>\n\n\n\n<p>Individuals with CAI often experience persistent swelling, pain, weakness, reduced ankle range of motion (ROM), ankle instability, diminished self-reported function, and recurrent ankle sprains.<sup>7,8<\/sup><\/p>\n\n\n\n<p>CAI has been identified as a precursor for ankle osteoarthritis (OA), and onset usually occurs a decade earlier than knee or hip OA.<sup>9<\/sup><\/p>\n\n\n\n<p>Chronic ankle sprains may require surgery in the form of arthroscopic ligament reconstruction. Individuals with CAI have been identified as having mechanical instability, relating to structural changes around the ankle, and functional instability which has been associated with a reduction in sensorimotor and neuromuscular control.<sup>10<\/sup><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Bracing for chronic ankle instability<\/strong><\/h2>\n\n\n\n<p>An ankle brace is a garment designed to stabilize the ankle before or after injury. Available in soft or semi-rigid types, these braces are designed to perform some or all of the following functions:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Improve ankle stiffness and thus mechanical stability<sup>11<\/sup><\/li><li>Improve neuromuscular control<sup>12<\/sup><\/li><li>Improve grounding of the foot<sup>12<\/sup><\/li><li>Decrease excessive ROM<sup>11<\/sup><\/li><li>Enhance proprioceptive acuity (the body\u2019s ability to sense its own location, movement, and actions)<sup>13<\/sup><\/li><\/ul>\n\n\n\n<p><strong>Several clinical studies have been carried out to determine the effectiveness of ankle bracing in these areas.<\/strong><\/p>\n\n\n\n<p>Hals et al.\u2019s 2000 study showed the Aircast<sup>\u00ae<\/sup> <a href=\"https:\/\/www.djoglobal.eu\/en_US\/Sport-Stirrup-73726.html\" target=\"_blank\" rel=\"noreferrer noopener\">Sport Stirrup<sup>\u00ae<\/sup><\/a> significantly improved shuttle-run performance in subjects with post-acute, unilateral, mechanically stable but functionally unstable ankle sprains.<sup>14<\/sup><\/p>\n\n\n\n<p>Habadi et al. (2014), demonstrated that soft and semi-rigid ankle orthoses are beneficial for dynamic balance of individuals with functional ankle instability.<sup>15<\/sup><\/p>\n\n\n\n<p>In their 2020 systematic review, Reyburn and Powden found that the current literature supports a strong effect of ankle braces on the dynamic balance of those with CAI.<sup>16<\/sup><\/p>\n\n\n\n<p>In 1998, Vaes et al. discovered the Aircast <a href=\"https:\/\/www.djoglobal.eu\/en_US\/Air-Stirrup-Classic-74580.html\" target=\"_blank\" rel=\"noreferrer noopener\">Air-Stirrup<sup>\u00ae<\/sup><\/a> brace significantly decreased talar tilt of unstable ankles in static and dynamic test conditions and slowed down the simulated sprain speed.<sup>17<\/sup> <\/p>\n\n\n\n<p>A randomized controlled trial by Janssen et al.
